APWU Health Plan is terminating the APWU Health Plan Management Staff Pension Plan (“the Plan”) effective April 30, 2026. As part of this termination process, all active and terminated vested participants are being provided with the opportunity to receive their Plan benefit as a lump‑sum distribution or as a monthly annuity. If you choose not to begin your benefit at this time, the Plan will purchase an annuity contract from an insurance company to provide your benefit in the future.

To assist you in returning your completed Plan termination election forms, we have established this online secure portal in association with Bolton Partners, Inc.. Click the button below to upload your completed election package. You will receive an email within 1 to 2 business days confirming that the Service Center has successfully received your forms.

To verify your identity, include a copy of your driver’s license when uploading your documents.
